General Data Protection Regulation (GDPR)


We would like to inform you that we have updated our Privacy Policy.


The updated policy will take effect immediately, in preparation for the General Data Protection Regulation (GDPR),

a new data protection law that comes into force on 25 May 2018.


We will continue to hold and use your personal data responsibly, and the update will help you to understand more clearly why we collect information, and how we use it.




We expect to receive flu vaccinations in mid-September 2019.

Please call the Surgery nearer this time, to see if they have arrived, and to book an appointment if you are eligible.


We have two different adult flu vaccines which have been recommended by Public Health England.

The Southgate Surgery has ordered a flu vaccine for you, to protect you against influenza.

  • This will also protect those around you, who may be more vulnerable to illness
  • Unlike a Pharmacy, we know our patients well
  • We hold your medical records, so that on the day that you attend the clinic, our Practice Nurses will be able to check that you are eligible to have the flu vaccination 
  • Having your flu vaccination done at The Southgate Surgery is free of charge

If you are aged over 65 years, you will have the trivalent vaccine (aTIV).

If you are aged 18 to 64 years, have a chronic condition eg diabetes, COPD, asthma, or CHD, or are pregnant, you will have the quadrivalent vaccine (QIV).

Please let us know if you do not want a flu vaccination, so that we can update your records, and offer the vaccination to somebody else.



Pre-bookable appointments only - 8.00am - 11.30am




All patients wishing to register for EMIS on-line services (which is now called PATIENT SERVICES) which will allow you to request repeat prescriptions and make / cancel appointments on-line, can attend on any day (Monday to Friday) between 8.00am and 6.30pm.


Please bring your photo ID to Reception and you will be given a Registration Letter

which will allow you to access Patient Services on-line.

It will support the Practice if you can make your appointments on-line. 

It will take some pressure off our telephone lines,

and make it easier for other patients to contact our Practice by phone.

Patients can now apply for on-line access to view detailed coded information from their medical records and we hope that our clinical software will be ready to support this function in due course. This will be in addition to the current on-line services listed below:-

we won't be able to register you at this Practice and will ask you

to find another local GP.

Please note that we will only register patients who live within our boundary, so please call us to see whether your address is within our catchment area. 


If you are already registered with us but move house and then live only slightly outside our boundary, you will be asked to re-register with another local Practice.  This is because we are having to make room for the ever increasing number of new patients moving into this area, particularly those living in new housing developments.  Your understanding will be appreciated.


If your address is within our boundary, you will be able to register at this Practice on Monday – Friday 11.30am and 6.00pm. 


When registering, please bring:- 


  • Your NHS number  (if you don't have your NHS number, you can obtain this from your previous GP) – this is not compulsory but will enable us to obtain your previous medical records more quickly
  • Proof of address eg utility bill, driving licence, bank statement, tenancy agreement
  • Please ensure that any handwritten information is easy to read  
  • When registering a baby or child, please bring their Red Book

It is in good practice for you to attend a registration medical with our HCA (Health Care Assistant) or Practice Nurse and you will be offered an appointment to attend.
