RESEARCH STUDY PARTICIPATION (SUMMIT STUDY)

 

University College London (UCL), and University College London Hospitals NHS Foundation Trust (UCLH) have embarked upon the largest ever lung cancer screening project in the UK.

 

WHO WILL BE INVITED?

People from this Practice aged 50 to 77.

 

WHAT IS INVOLVED?

  • A letter will be sent to you by your GP with a phone number to call
  • Depending on your responses to some questions over the phone, you may be invited to attend a Lung Health Check (where your lung health will be assessed); or you may be invited to attend a study appointment in your local area
  • You may be offered a scan of your lungs (called a CT scan), and be asked to give a blood sample, as part of the study; or you may be asked to give a blood sample and complete a questionnaire
  • The study is entirely voluntary and you do not have to participate if you do not wish to do so

STAFF AT UCL AAND UCLH ARE ORGANISING THE APPOINTMENTS FOR THE RESEARCH STUDY

  • For this to happen, this GP Practice will securely share the names and addresses of potentially eligible individuals with the study team
  • The study team will invite people, and co-ordinate study appointments.  They will keep the details of everyone they invite for a period of up to 10 years, in order to collect important health-related information, and help develop health services for the future
  • Strict security measures will ensure your information is kept safe and confidential

IF YOU DO NOT WANT TO PARTICIPATE IN THIS STUDY, AND WOULD PREFER TO OPT OUT

  • Please contact us before Friday 20 September 2019
  • Please call our Results Line (tel 020 8440 9301 option 2 - Monday to Friday 2.00 to 4.00pm)
  • Opting out WILL NOT affect any of your future care
  • You will continue to be invited for other NHS appointments and activities

General Data Protection Regulation (GDPR)

 

We would like to inform you that we have updated our Privacy Policy.

 

The updated policy will take effect immediately, in preparation for the General Data Protection Regulation (GDPR),

a new data protection law that comes into force on 25 May 2018.

 

We will continue to hold and use your personal data responsibly, and the update will help you to understand more clearly why we collect information, and how we use it.

FLU CLINICS 2019


We are still giving flu vaccinations for patients in the following groups:-

  • Over 65s
  • Patients aged 18 to 64 with a chronic condition eg Diabetes, COPD, Asthma, or CHD
  • Pregnant ladies

If you are in one of these groups, and would like a flu vaccination, please contact us to make an appointment.

MAKE / CANCEL YOUR APPOINTMENTS, AND REQUEST REPEAT PRESCRIPTIONS ON-LINE WITH EMIS PATIENT SERVICES

 

All patients wishing to register for EMIS on-line services (which is now called PATIENT SERVICES) which will allow you to request repeat prescriptions and make / cancel appointments on-line, can attend on any day (Monday to Friday) between 8.00am and 6.00pm.

 

Please bring your photo ID to Reception and you will be given a Registration Letter

which will allow you to access Patient Services on-line.

It will support the Practice if you can make your appointments on-line. 

It will take some pressure off our telephone lines,

and make it easier for other patients to contact our Practice by phone.

Patients can now apply for on-line access to view detailed coded information from their medical records and we hope that our clinical software will be ready to support this function in due course. This will be in addition to the current on-line services listed below:-

Access to information about medications / allergies


Order Repeat Medication Online


Routine repeat prescriptions can be issued on receipt of a written request, without having to see a Doctor. These requests should either be placed in the box at Reception, or posted to the Surgery. If you enclose a stamped addressed envelope, the prescription will be posted to you. 

 

Requests which arrive before 12 noon are usually available to collect from the Surgery 48 working hours later. Please make sure that you allow enough time for possible postal delays.

 

All patients will be on our computer repeat prescription system, and we ask that the instructions on the prescription ‘tear off’ portion are observed, ie. to tick the drugs required.

 

From time to time, the Doctor will require you to make an appointment to ensure the continuation of your prescription. When this is due, the computer slip will tell you to ‘see Doctor this time’. An appointment should be made well in advance of your supply running out.

 

Our Practice Policy states that it is not possible to accept telephone requests for repeat prescriptions.

 

However, if you have any questions regarding your prescription, we recommend that you call this Surgery between 9.00am to 1.00pm, Monday to Friday, and chose option 3, when somebody will be able to help you.

 

When medication is prescribed by a hospital, we can only issue a prescription for this medication once we have received email/written/faxed confirmation from the hospital. We do not transfer Private Prescriptions to NHS Prescriptions.

 

Order Your Repeats Online With EmisWeb

 

 

Once you have registered for Online Patient Services, you will be able to order repeat prescriptions online.

 

Show your photo ID  such as a passport or driving licence, to a Receptionist.  You will be given details of how to register.

 

 

Electronic Prescription Service

 

Register for the Electronic Prescription Service so that your prescription can be sent directly to your chosen pharmacy or chemist.  Once you have completed your prescription request (preferably online), you can collect your medication from the pharmacy or chemist, without the need to collect a paper prescription from the Surgery.

 

Complete a form today (available from Reception) and hand it to either our Reception staff or to your pharmacist / chemist.
